Obesity involves changes in the function of brain Areas that Govern body fatness and blood Sugar, particularly an Area called the hypothalamus.  

My coworkers and I showed that obesity is associated with inflammation and trauma of the hypothalamus in rodent models, and we all presented preliminary evidence that the same could be true in humans.   Within our latest paper, we confirm this association, also show that hypothalamic injury is also connected with a mark of insulin resistance, independently of BMI.
